Mandatory Enrollment Forms
We ask parents to fill out all mandatory forms listed below for children starting attending Dyvosvit School and Childcare. All filled forms should be handed to the Dyvosvit administrator on the first day of attendance, except JFS 01305 form*, which should be provided no later than 30 days from the attending start.
* JFS 01305 form is not needed for school-age children attending afterschool programs during the school year or camps during the school vacations and in the summertime.
Parents who are employed, in school, or participating in meeting the requirements of the Ohio Works First (OWF cash assistance) or Supplemental Nutrition Assistance Program (SNAP) can apply for publicly funded child care(PFCC) benefits to help to pay for their child care.

If you need to transfer your child, who currently receives PFCC assistance, to Dyvosvit School and Childcare from a different child care provider, you may do that by completing Cuyagoha County JFS office's Childcare Change Request Form.
Families with an active Ohio Benefits account for other programs (Cash, Food, or Medical assistance programs) can add a new PFCC application to their existing SSP accounts.
Families should not apply for initial PFCC eligibility if they have already done so and are receiving PFCC benefits.
Using the SSP allows families to view their application status and easily report changes, including a child care provider change, all from one account.
Dyvosvit School and Childcare is a licensed program that can provide a caretaker and child with regulated, quality care through state and federal requirements monitored by ODJFS for compliance. This include health and safety, staff Professional Development training, CPR and First Aid, background checks, and additional monitoring inspections, as needed.
In addition, children will receive additional early learning supports through the participation of Dyvosvit School and Childcare in Ohio’s 5-star Quality Rating and Improvement System, Step Up To Quality. Step Up To Quality recognizes learning and development programs that exceed licensing health and safety standards, family and community engagement, health promotion, staff professional development, and child development standards.
If you are approved for Publicly Funded Child Care, you may still be required to pay for part of your child care, called co-payment. Your co-payment is based on your income, family size, and how many children you have in child care. You can get more information about the child care program from your County Department of Job and Family Services (CDJFS).
